If you are a current or potential missionary applying as a candidate for financial support, we ask that you please take the time to complete our questionnaire.  This is the first step in our process for considering a missionary for support.  We understand that questionnaires can be a bit "impersonal" and are seen as offensive to some.  That is not our intent at all.  We are not trying to be offensive, picky or tricky: we're are simply trying to find out where you stand on certain issues that are important to us when considering missionaries for support.

* Please share your testimony of salvation
* Please discuss how you believe a person is saved. In the space provided, please explain how you would lead a person to salvation (simply give us your presentation of the gospel).

* Berean uses wine in the Lord's Supper, although we support missionaries who use grape juice. Would your church support a missionary who uses wine in the Lord's Supper?

* If you are sent with a missions organization, does/would your church support a missionary that is not sent under that same organization? Explain why or why not.
